I know that there are some information missing. I'll provide them asap. I think with that information you are able to start with coding.

Furthermore I think I found a good solution to support you with the code. I'm going to do the styles for buttons, input fields, etc. So I'll try to provide the CSS markup for that elements. And I would like to try to make some fine tuning when you are almost finished.

Only cosmetic reasons. I'll review a custom grid before if it works flawlessly. Perhaps it is not worth. We can skip that for the Captive Portal and adapt it later if it works well.
